Within this form you will be able to familiarize and discuss all aspects involved with our “Worry Free” Cremation. No matter where you are, our authorization can be conveniently downloaded, signed and returned by email or fax (electronic transmission) with a copy of signers photo identification.

The general price list contains itemized information specific to goods and services provided by the funeral home to its consumers. Consumers also have the right to know the legal requirements and important disclosures explained in our General Price List.

Georgia Cremation strives to serve every Veteran and their family members with the honor, dignity and respect they have rightfully earned. Any veteran can choose to claim a United States Flag for burial purposes, which will be presented to the closest relative during the service.
Georgia Cremation Claim For Medallion »
For veterans who choose to be buried private cemeteries, a Government Medallion can be claimed for placement at the gravesite. All medallions will come with the word “Veteran” inscribed at the top, and the Branch of Service at the bottom. It is a fitting identification marker for those who chose a personal headstone or marker.
Veterans Administration Burial Benefits Form »
While we have provided this VA Form 21530 to assist veteran’s families in filing for burial benefits, Georgia Cremation files all applicable Veterans Administration benefit forms. Some additional forms include: VA Form 400247 Presidential Memorial Certificate Request Form, VA Form 272008 Application for United States Flag for Burial Purposes and VA Form 401330M Claim For Government Medallion for Placement in Private Cemetery. Additionally, Georgia Cremation will coordinate the necessary request(s), details and services with all National Cemeteries and Honors Detail teams from each branch of the U.S. Armed Forces.
